Transition Co-ordinator visits the college weekly and is in regular contact with majority of students. Transition Co-Ordinator is also in contact with Welfare Officer at the college and receives regular updates on how students are progressing.

Student A: Moved out of area and then returned to Medway. Social worker is supporting.

Student B: Is under YOT. Assigned team worker is supporting with employment/ training.

Student C: Transition Co-ordinator is supporting as student has been applying for jobs/ apprenticeships to no avail.

Student D: Transition Co-Ordinator has been unable to make contact with student and has referred to NEET programme.
